Individual Notification Form

Individuals should report any prohibited conduct (as defined in the Simons Foundation International’s grant code of conduct) that impacts a grant funded by Simons Foundation International to the designated persons at their institution.

If the institution does not address the report, individuals who have concerns may contact Simons Foundation International through this web form. In that case, Simons Foundation International may follow up with the institution, as appropriate, to confirm that the grant is being performed in a workplace consistent with its grant code of conduct.

This is a secure web form, and all submissions are encrypted and reviewed by authorized staff.

Personally identifiable information regarding any complainants or other individuals involved in the matter must not be included in the notification.
